Space Maintainers: Why Premature Loss of Milk Teeth Needs Protection

Dr. Bhagyalakshmi Avinash 5 Min Read Clinically Reviewed
When a baby tooth has to be extracted early due to severe decay or sports trauma, parents often assume the space will simply wait for the permanent tooth to arrive years later. In reality, teeth behind the empty space naturally shift forward within weeks, permanently locking out the adult tooth. A space maintainer is a simple, affordable device that prevents this disaster.

1. What Happens When Space is Lost?

The natural tendency of posterior teeth is to migrate forward (mesial drift). If a baby molar is lost at age 6 or 7 and the adult premolar isn't scheduled to erupt until age 10 or 11, the 6-year permanent molar drifts forward into the empty socket.

By the time the adult premolar tries to come in, its space is gone. It either erupts sideways out of the gum, gets impacted inside the jawbone, or causes severe crowding requiring multi-year braces or adult extractions.

2. Types of Space Maintainers at Dental Planet

  • Band and Loop (Fixed): A tiny stainless steel ring fitted around the adjacent molar with a polished wire loop resting against the front tooth to hold the space open.
  • Crown and Loop: Placed when the adjacent anchor tooth also requires a protective pediatric crown.
  • Lingual Holding Arch: Placed behind the lower front teeth when multiple baby molars on both sides have been lost.

3. Painless Placement and Easy Living

Space maintainers are custom-made from digital scans. They are cemented with dental fluoride cement in under 15 minutes with zero drilling and zero pain. Children eat, speak, and play normally without even noticing them.

Frequently Asked Questions

Q: When is a space maintainer removed?

Dr. Bhagyalakshmi monitors the child with X-rays. As soon as the permanent adult tooth starts breaking through the gum, the space maintainer is simply popped off in seconds.

Q: Can space maintainers trap food?

Regular toothbrushing easily cleans around the wire loop. We advise avoiding sticky chewing gums or toffees that could pull the band loose.
